Parent/Guardian Consent

By payment of the Association affiliation fee, parents/guardians consent to the following:

  • Information provided on this form may be used by the Calisthenics Association of WA (“the Association”) for the administration of the sport of  calisthenics and in accordance with the objects of the Association. This information will be held in an Association database at the Association office and may be forwarded to the Australian Calisthenics Federation (“ACF”) for use in the administration of the sport of calisthenics on a national level and in accordance with the objects of the ACF. Members can access their personal information through the Association upon request.
  • The member’s name and photograph (but not address) may be recorded and/or published in any form of CAWA and ACF media including video, live streaming, website, social media, newsletters, reports and programs (whether printed or electronic) for the purpose of calisthenics.
  • Photos during competition presentations may be taken and published by the audience.
  • Video may be recorded of the member whilst training and during Association (or club) performances. Such videos may be used for:
  • family keepsakes by family members or other members.
  • by or for coaching staff to be used for coaching purposes.
  • by or for adjudicators for adjudication purposes.
  • Information about calisthenics may be sent to the member from time to time by mail or email.

Authorisation may be retracted at any time; such retraction to be submitted in writing and forwarded to CAWA Administration, via email at info@calisthenicswa.com.au or PO Box 1464, Midland DC WA 6936.

Participant Code of Conduct

PARTICIPANT’S AGREEMENT

In Registering as a member of Carine Calisthenics Club, the participant agrees to the following requirements in regard to their conduct during an activity held by or for Carine Calisthenics Club.

The participant will:

  • Respect the rights, dignity and worth of others including fellow participants, coaches, officials and spectators.
  • Be fair, considerate and honest in all dealings with others.
  • Be professional and accept responsibility for all actions.
  • Refrain from conduct which could be regarded as bullying, harassment, discrimination or abuse towards fellow participants and coaches including in person, by email, over social media (e.g., Snapchat, Instagram, Facebook etc) or any other form of communication.
  • Care and respect any equipment provided by to them by Carine Calisthenics (e.g., costumes, headgears).
  • Conduct themself in a professional manner relating to language, temper and punctuality.
  • Place any smart devices such as phone or smartwatch in the allocated storage space at the start of every class upon entering the hall.
  • Maintain high behavioural standards at all times.
  • Abide by the rules and respect the decision of adjudicators.
  • Cooperate with coaches and personnel.
  • Be honest in their attitude and preparation in training. Work equally hard for themself and the team.

PARENTAL AGREEMENT (if participant under 18 years)

The parent/guardian agrees to the following requirements in regard to their conduct during an activity held by or for Carine Calisthenics Club.

The parent/guardian will:

  • Respect the rights, dignity and worth of others including participants, coaches, officials and spectators.
  • Be fair, considerate and honest in all dealings with others.
  • Be professional and accept responsibility for all actions.
  • Refrain from conduct which could be regarded as bullying, harassment, discrimination or abuse towards participants and coaches including in person, by email, over social media (e.g., Snapchat, WhatsApp) or any other form of communication.
  • Conduct themselves in a professional manner relating to language temper and punctuality.
  • Be frank and honest with the coaches concerning injury and/or illness and how it may affect the abilities of their child at both classes and competitions.
  • Maintain high behavioural standards at all times.
  • Communicate any questions/issues directly to the team liaison.
  • Abide by the rules and respect the decision of adjudicators.
  • Cooperate with coaches and personnel.

In acknowledging these terms of registration, the participant and parent/guardian (if the participant is under 18 years of age) have reviewed, understand and will abide by the contents of the Participant Code of Conduct.

The participant (and parent/guardian where applicable) acknowledges that failure to meet the standards set by Carine Calisthenics Club outlined above will result in a review of behaviour by the Carine Calisthenics Club Committee. We also acknowledge that in the event of a breach of this Participant Code of Conduct or any team-specific Code of Conduct (where applicable), that this acknowledgement may be used as evidence of your agreement to maintain teh Participant Code of Conduct.
